Disclosing your disability means that you notify your employer or potential employer of your disability.
Federal contractors now have a special form, known as the self-identification form, to invite job candidates and employees to disclose if they are an individual with a disability.
- Completing the form, or disclosing your disability in any way, is always voluntary.
The form is available online at http://www.dol.gov/ofccp/regs/compliance/sec503/Self_ID_Forms/SelfIDForms.htm.
For more information about making the choice to disclose your disability, review Chapter 4, Getting a Job.
